Eligible NJHS Student Information

Attention all qualifying candidates who have received an invitation letter in the mail, please return the Student Activity Information Form to the Counseling Center by 3:00 p.m. on Friday, March 13th. Click here to print the form.

Wash Kit Drive

The Peanut Butter Gang is sponsoring a toiletries drive to make washkits for struggling neighbors. We'll be collecting washcloths, soaps, and any small-sized personal hygiene products. If you're traveling over the vacation, those hotel-size shampoos and lotions would be perfect for our drive! There will be a box for donations in the counseling center lobby after the vacation. Thank you.

Spring Sport Tryouts begin March 23, 2020

In order to be cleared to tryout for a sport, all health and physical forms must be completed and returned to the Health Office before tryouts. These forms can be downloaded and printed from the school website under the Athletics Department tab where the documents are indicated.

The following forms are required:

Health Certificate/Physical Form - Both sides completed, signed and stamped by your child’s physician (this physical is good for one calendar year).

Health History/Medical Update Form - This is a one page form to be completed and signed by parent or guardian. This form must be handed in within 30 days of every try-out. This form must be dated after February 23, 2020.

Please drop off forms to:

Great Hollow Middle School, 150 Southern Blvd, Nesconset, NY 11767

To check your clearance go to: https://student.smithtown.k12.ny.us. Students enter their username and password. This is not the Parent Portal. (Please note passwords are case sensitive). If the student is not cleared, it will detail exactly what is still needed.
